With the Yankees trailing 6-3 to Houston in the bottom of top of the eighth inning in Game 4 of the ALCS, Aaron Boone called upon CC Sabathia out of the bullpen.

The left-hander made his second appearance of the series, but after suffering a shoulder injury, Sabathia walked off the mound at Yankee Stadium for what could be the final time of his career.

Sabathia allowed Yordan Alvarez to reach on an error, but then forced Carlos Correa to fly out to right. After hitting Robinson Chirinos with a pitch, Sabathia forced another flyout to Aledmys Diaz. Looking to get out of the inning, Sabathia threw a pitch to George Springer and immediately showed signs of injury.

Boone and members of the Yankees training staff came out to check on him, and after appearing to tell them he was okay, Sabathia threw one warmup pitch and turned to head towards the dugout.

The Yankee Stadium crowd gave Sabathia a loud ovation as he walked down the dugout stairs.

Boone said after the game that the team would need to wait for test results, but it's "not good" with Sabathia's shoulder. The team later officially announced a left shoulder injury for Sabathia.

"He leaves it all on the field. I don't know the severity of what's going on with him, but you can never say that man never gave his all," said Aaron Judge after the game. "Every single time he went out there, you had to rip the ball and rip his jersey off to get him off of that mound. He got everything out of that arm, and that's a warrior right there. That's why he's been a leader of this team for so many years and that's why he has so much respect around the game and in this stadium.

"It was tough to see, but that's our leader, and he left it all out there."

Sabathia, who has already announced that this season would be his last, has a career record of 251-161 with a 3.74 ERA. A six-time All-Star, Sabathia won the AL Cy Young award with Cleveland in 2007.
